On this page you will find the applications for license and permits available for download. Please follow the instructions below:
PC Users: To download a file below, simply right-click on the link, then choose "Save Target As" and save it to your computer.
Macintosh Users: Click on link and hold down the mouse button until a dialog box appears. Then, choose "Save This Link As" and save it to your computer.
After saving, open the file and print it. Then, complete the form and mail it to the address indicated on the form or to the address below.
